Summary of Essential Functions for the Position:
The Intake Coordinator receives incoming calls from patient referral sources, gathers necessary information, and
ensures quality communication between referral sources and admitting offices. Assist in answering and
dispatching phone calls and messages. Works with others in a team environment. Works in conjunction with
the sales team on establishing lines of communication with new referral sources and providing positive

Qualifications / Licensure / Certification / Knowledge / Skills / Abilities:
1-2 years experience in healthcare/homecare, preferred. Requires the ability to exchange routine information in
an appropriate manner requiring good oral and written communication skills. Previous administrative and
secretarial experience desired. Position requires good organization skills, time management, customer relation
skills, ability to multitask, and competent with computer applications required.
High school diploma or GED.
Working Conditions & Physical Requirements:
Works indoors in agency office. Most of the day is spent sitting. Requires minimal physical effort most of the
day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity
changes and minimal assist in lifting 20 pounds. Must possess sight/hearing senses or use appropriate adaptive
devices that will enable senses to function at a level required to meet the essential duties of the position.
