  • Overview

    The LPN/LVN Clinical Coordinator is primarily responsible for the timely processing of client referrals. S/he assists the RN Branch Manager in coordination of clinical services.
    I. Work Flow Necessary to Ensure Clinical & Financial Success (component proportion 75%)
    1. Completes the referral/intake process including but not limited to the initial contact with the patient, securing physician orders and coordination/documentation with sister Divisions, as well as other referral sources.
    2. Participates and assists direct care staff in the coordination of care, including but not limited to the reporting of
    abnormal findings, securing of physician orders, and transmittal of lab results.
    3. Completes assigned work flow tasks within timeframes allotted by the Division, including but not limited to, the processing of the unlisted events, review of entitlement verification reports, etc.
    4. Processes wound reports, investigates cause of significant score deviation and reports, as needed, to the RN
    Branch Manager.
    5. Participates in patient case conferences and documents ongoing Coordination of Care with other members of the healthcare team.
    6. Assists the Branch Manager and marketing team, as needed, in marketing efforts.
    7. Performs duties and responsibilities of the field LPN/LVN, as needed.
    8. Job Responsibilities as aligned with HH Condition of Participation 42 C.F.R. 484.75(b) published in Federal Register
    a) Ongoing interdisciplinary assessment of the patient;
    b) Providing services that are ordered by the physician as indicated in the plan of care;
    c) Patient, caregiver, and family counseling;
    d) Patient and caregiver education;
    e) Preparing clinical notes;
    f) Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;
    g) Participation in the Home Health Agency’s QAPI program; and
    h) Participation in Home Health Agency’s-sponsored in-service training
    II. QAPI Activities/Home Health Compare Reports (component proportion 10%)
    1. Assists with review of clinical documentation, as assigned by the Branch Manager.
    2. Participates in the QAPI process of the agency, seeking to identify opportunities for performance improvement.
    3. Reports significant patient findings to the RN Branch Manager and collaborates on actions or interventions to be taken.
    III. Program Administration Coordination (component proportion 15%)
    1. Responsible for the appropriate use of agency resources including, but not limited to, supplies and personnel to ensure financial success.
    2. Shares on-call responsibilities, as scheduled.
    3. Works to maintain positive, collaborative relationships with patients, caregivers, referral sources, and/or physicians.
    4. Maintains current knowledge of nursing practice and is responsible for personal continuing education required
    by the BNE.
    5. Participates in internal staff meetings and training activities, as assigned.

    6. Collaborates with members of the marketing team to assist in identifying potential referral sources, and patients appropriate for home care.
    7. Performs all other duties as assigned.
    I. Education
    In accordance with licensing regulations, must meet the following criteria
    Must be a Licensed Practical/Vocational Nurse (LVN) in the state of practice or in accordance with the Board
    of Nurse Examiners rules for Nurse Licensure Compact (NLC). II.
    II. Experience
    1. Minimum of two (2) years of active experience as a LVN, preferred. Louisiana only: must have worked at least one year as an LPN prior to being employed by a home health agency.
    2. Possession of a working knowledge of Medicare/Medicaid home health care benefits, policies and procedures preferred.
    III. Skills & Abilities
    1. Possesses excellent customer service skills.
    2. Has a positive attitude and willingness to learn throughout employment.
    3. Self-starter with excellent verbal and written communication skills.
    4. Ability to work with both clinical and non-clinical staff members.
    5. Willingness to learn and utilize software solution, inclusive of Divisional Work Flow Tasks.
    6. Must be organized and have the ability to manage multiple tasks & priorities at any given time.
    7. Possesses a firm foundation of basic nursing skills.
    8. Willingness to learn home health regulatory standards related to job-specific duties, including but not limited to: OASIS, Condition of Participation, state-specific licensing rules, diagnosis coding, and internal policies.
    IV. Other
    1. General
    A. Must participate in the Coordination of Care.
    B. Must have satisfactory references from previous employers.
    C. Must be able to pass a background check which may include criminal history check and OIG sanction screening.
    D. Must be willing to submit to and be able to pass a random drug screening.
    E. D. Must have a dependable vehicle, a valid driver’s license, and maintain vehicular insurance in accordance with laws of the State.
    Louisiana only:
    F. Must inform all employers when employed with one or more agencies and cooperate and coordinate to assure highest performance of quality when providing services to the patient.
    2. Risk Exposure (bloodborne pathogens, etc.) A. Moderate to High Risk.
    3. Physical Requirements
    A. Heavy lifting.
    B. Ability to participate in physical activity.
    C. Ability to do extensive bending, lifting and standing on a regular basis.
    D. Ability to work for extended period of time while standing and being involved in physical activity.
